Breast Augmentation with the Preserve Technique: Innovation & Natural Results

Breast Augmentation with the Preserve Technique at Cirugía Plástica con Sentido, located in Guadalajara, Mexico, we are dedicated to aesthetic, reconstructive, and breast surgery, with over 15 years of expertise led by Dr. Juan Gordillo. Our practice is based in the Hospital Puerta de Hierro Sur. In this blog post, we explain the Preserve breast augmentation technique: procedure, ideal candidates, anesthesia, and its benefits.

What is the Preserve technique in breast augmentation?

The Preserve technique (or “preservative” method) is an advanced augmentation mammoplasty that aims to maintain as much of the natural breast anatomy as possible—the muscle, connective tissue, and fascia—by creating a subfascial or intermediate pocket for highly cohesive silicone implants. The aim is to add volume and projection while preserving function and stability of mammary tissue.

During the procedure, careful incisions are made (commonly inframammary, periareolar, or sometimes transaxillary), a gentle dissection is done into the subfascial plane, the implant is inserted, and drains or fixation may be placed as needed. Symmetry and aesthetic harmony are emphasized, respecting surrounding structures.

Who are ideal candidates?

  • Women aged 18 years or older, with completed breast development

  • Who want to increase breast volume for aesthetic or reconstructive reasons

  • In good overall health, without contraindicating illnesses

  • With realistic expectations and willingness to comply with post-op care

  • Not pregnant or breastfeeding

In the initial evaluation, we perform mammogram or ultrasound, assess the quality of skin and glandular tissue, and the breast position.

Anesthesia: sedation or general

Anesthesia choice depends on the case. For more straightforward surgeries and low-risk patients, sedation plus local analgesia (sedoanalgesia) may be used. In more complex procedures or when combined surgeries are involved, general anesthesia is preferred. Our anesthesia team continuously monitors vital signs to ensure safety and comfort.

Advantages of the Preserve technique

  • Anatomical preservation with minimal muscle disruption

  • Less implant animation when activating chest muscles

  • Natural-looking results with lower risk of capsular contracture

  • Discreet scars and refined aesthetics

  • Often more comfortable recovery compared to more invasive techniques
